Fees

The initial session with Dr. Keith is $185.00 and typically lasts 45-50 minutes.  Following the initial session, Dr. Keith charges $150.00/session for individual therapy and $165.00/session for couples and family counseling. 

Assessment fees range between $300 and $2,000, depending on the nature of the evaluation.  Dr. Keith will be able to provide you with an exact fee after discussing your needs.

Consulting and Coaching fees are $200/hour.  Dr. Keith typically meets with consulting and coaching clients in their place of business.


Insurance

Dr. Keith is currently an "in-network" provider for Aetna, Humana, Blue Cross/Blue Shield, and LifeSynch.  Coverage depends on your individual health plan.  As insurance plans vary, it is best to call the office and speak to the office manager to determine your benefits.
 
If you are not insured with any of the above plans, Dr. Keith can help you determine your "out-of-network" benefits and file with your insurance company on your behalf.  Many insurance companies reimburse between 50-80% of the session cost for an "out-of-network" provider.


Payment Options

Dr. Keith accepts cash, personal checks, MasterCard, and Visa. Payment is due at the time services are rendered.


New Patients: Prior to your first appointment with Dr. Keith, print, read, and complete the applicable forms below. Please bring the completed paperwork, your driver's license, method of payment, and insurance card (if applicable) with you for your first appointment.

New Psychotherapy Patients please complete both the Psychotherapy Services Agreement and HIPAA form.

New Assessment Patients please complete both the Psychoeducational Assessment Agreement and HIPAA form.
